1. Selects an internship/field work experience site in consultation with the faculty advisor.

2. Makes arrangements to interview with the potential site supervisor.

3. Completes and submit the Health Informatics Internship/Field Work Experience Form.

4. Completes the internship/field work experience requirements as stated in the Health Informatics Internship/Field Work Experience Form.

5. Meet with the health informatics faculty advisor at the end of the first 90 hours. At this meeting, the student will be expected to give the faculty advisor a progress report.

6. Coordinate and be present during the faculty advisor's site visit.

7. At the conclusion of the internship period, the student will meet with the faculty advisor and will submit the following:

• Log of activities
• Project report
• Evaluation of Site Form
